When you register the Donate Life special issue license plate to your car, the National Kidney Foundation of MA, RI, NH & VT will receive a percentage of the fees paid. 

To get started, the NKF is required to collect 1,500 applications for the plate before any plates are actually manufactured. In addition to receiving the application you can download here, the NKF must collect a $40 special plate application fee. Please make your check payable to the Registry of Motor Vehicles. We’ll hold onto your check until all 1,500 applications are filed, then we’ll present them to the RMV to process payment. Of the initial $40 special plate feel, $28 will be donated to the NKF. Registration is valid for two years. At renewal, the entire special plate fee goes to the NKF to develop programs and services to promote organ and tissue donation and transplantation.
